How
do I reinstate a lapsed policy?
-Fill-out a REINSTATEMENT QUESTIONNAIRE form
for a policy that lapsed within 90 days from its last premium
due date or an APPLICATION FOR REINSTATEMENT
and HEALTH DECLARATION form for a policy
that lapsed beyond 90 days but not more than 3 years from
the last premium due date
-Pay the PREMIUM ARREARS and OVERDUE INTERESTS
When
can I avail of a policy loan and what are
the requirements?
-Loanable amount is available @ the start of the 4th policy
year for PARTICIPATING PLAN or @ the start of the 3rd policy
year for NON-PARTICIPATING PLAN
-Submit a duly completed and notarized APPLICATION
for POLICY LOAN form
-Present an Acceptable Identification Card, i.e. PRC ID, Company
ID, Passport, SSS ID, etc...
-Loan check is released within the day or the following day
How
much is the minimum coverage to avail of the free annual medical
check-up with breakfast?
- 1 Million if your policy is classified as a Permanent plan
or 5 million if your policy is a Term Plan
What
are the requirements if I wish to avail of my free annual
medical check-up thru home service?
- you must have a minimum sum insured of 4million for permanent
plans or 10million for a Term plan

What
is the consequence if I fail to elect a
NON-FORFEITURE option in my application
for insurance?
- As approved by the Insurance Commission, the company will
apply the following options:REDUCE PAID-UP (RPU) for policies
issued prior to December 31, 1998 EXTENDED TERM INSURANCE
(ETI) for policies issued after January 1, 1999
What
are the basic steps in filing a death claim?
-Inform the Customer Service Department of the death of the
Life Insured
-Accomplish the following forms available @ First Life's Head
Office

CERTIFICATE
OF CLAIMANT/BENEFICIARY form
•
CERTIFICATE
OF ATTENDING PHYSICIAN form
•
DEED
OF SETTLEMENT-DEATH CLAIM form
-Then, submit the above documents together with:
1. Certified true-copy of Death Certificate
2. Original copy of Policy Contract or Affidavit of Lost Policy
form
3. Birth Certificate of the Deceased Insured
4. Birth Certificate of the Claimant/s /Beneficiary/ies
5. Marriage Contract, if applicable
6. Police Investigation Report, if any
7. Obituary or newspaper clippings, if any
