How do I change my BENEFICIARY/IES?
-Fill-out a CHANGE of BENEFICIARY form and have the irrevocable beneficiary/ies sign on the form, if there is any I lost my original policy contract, how can I get a replacement?
- Fill-out an AFFIDAVIT of LOST POLICY form and have this form notarized
- Pay the POLICY FEE

What are the requirements for a change in civil status?
-Fill-out a REQUEST for AMENDMENT form
-Submit any Proof of Change, i.e. Certified True-Copy of Marriage Contract, Court Order, etc...
